Before you begin the worksheet: Ask your instructor which file type they prefer.  Then choose one of the file formats below (PDF or Word).  Before you begin, you must save the document to your computer.  After you save it initially, you can then open it, fill it out, and save it again under a unique file name.  If you do not save the document to your computer first, you may lose your work!
